Where to Go
Visit the Rockford Housing Authority office during regular business hours (Monday – Friday, 8 AM – 5 PM). You can request an , get help filling it out, or submit your completed form directly to our intake team.
What to Bring
Bring as much documentation as possible: photo ID, Social Security cards, income proof, birth certificates, and residency verification. Staff will guide you on what is required and help you fill in any missing details.
